How do you create this kind of motion blur in portrait?

There is like a nice line around the subject...How do you create this?

Moon probably put a strip of transparent tape that was a third of the width of the front element of her lens vertically across it. She may have directly taped a filter, or, heck, maybe even applied the tape to the front element. The tape produced both the blur and the off-axis double image.

Honestly, it looks like she just duplicated the frame, dropped the opacity on the top layer, and nudged it over.
