The motion blur was created in camera with minor Photoshop adjustments to background.
I used a three-light setup to achieve the results. Two strobes were behind subject at 120º from camera. The third light was about 60º to the right of the camera. Einstein strobes used for shoot.

Nice one!! Something a bit different from a shot that has been done 'million times' - like the motion blur and fact that it's not all Photoshop manufactured.
One tiny bit of me would have liked to see a bit of the court light up.. just to give more sense of the height of the jump.
Ville, thanks for the compliment and your feedback. I actually used a large black background to ensure that the background would be black. I wanted the motion blur to show through as much as possible. Thanks for checking out the image.
