This Photo is taking outdoor, smoke from the mouth and the background is absolutely real. I used a 100 cm octobox on the right side and a kick light on left, behind smoke.
Everything was very quickly because smoke held 15- 20 seconds.
I used Canon 5d mark III , sigma 85 mm, f/2.5 , 1/160 sec , iso 320 and retouching technique in photoshop cs 6 with frequency separation.

Hello, It was around 8 o'clock PM, ambient light did not affect photo,it was low light. Picture was taken in the forest, because I could not let smoke in studio. I created the same conditions as in studio.
