Normally I would suggest turning down your modelling lights, to slightly dilate his eyes, but in this case the staring eyes effect adds to the crazy, excellent.
It looks sightly over exposed and too blue and I would not mind a little more shaddow, just a very subtle increase would be enough, a slightly darker background would focus the viewer on the face even more than the crazy expression does already, adding impact. Just personal preferences, not complaints.

Now that is one crazy picture, I like it... I agree with Ian that it seems a bit blueish, may want to add a few 100 Kelvins to the WB :)
