Nikon D800, 24-70mm f2.8, 2 light setup with AlienBees B800's, a round reflector and a white 3 fold posterboard underneath the model. 1 B800 setup in a 22" beauty dish and the other B800 setup directly behind the subject with a bare bulb and 7" reflector to blow out the background.

Nice work but I feel like if you're going to crop the top of the head, maybe have the model not have her glasses up there. Also, for such a high-key image, maybe a lighter colored top or at least one with a less interesting pattern if the focus is supposed to be on her face.
