I love the story this image communicated. Really painterly with lovely tones. I very much appreciate dark painterly work.
The only thing that I personally find distracting are the bluish, magenta, and yellow/orange (?) colour patches on the skin. Unless it's intended, and perhaps it is for artistic reasons, I'd try to at least soften them.
