I shot this image with a Canon 7D Mark ii, canon 75-300mm 4.0
1/200s, f10 ISO 100
It's a 2 lights setup

I used a 7feet parabolic umbrella as background to give me the soft seemless white background
My keylight was more from my camera left
I had a mirror at my camera right to fill in the shadows and to create the fancy catchlight

Retouched with camera raw and photoshop

Solid image, however, I feel like you oversharpened it a little. The skin would look nicer, if you actually blurred it a little, or used a frequency separation to smooth it.
